Updates to ISSDA Data Request Forms

ISSDA have updated our Data Request Forms and End User Licence. We have also introduced a Confirmation of Data Destruction Form upon project completion.

ISSDA Data Request Form for Teaching Purposes - Pseudonymised Datasets

The main changes to the forms are outlined below - 

  • Only institutional email addresses will be accepted.
  • Where the Lead Researcher is a PhD, Masters or Undergraduate student the End User Licence must be countersigned by their Supervisor, who has the responsibility to oversee their adherence to ISSDA’s conditions.
  • The End User Licence must be signed by a representative of the Research Organisation. This includes but is not limited to Head of School/College/Faculty/Department, Head/Dean of Research/Research Support. By signing, they acknowledge and accept the statements within the agreement and confirm that they are duly authorised by the Research Organisation to enter into the End User Licence.
  • Contact information for all of the Research Team,  individuals who will access or use the data, must be given.

ISSDA have also introduced a Confirmation of Data Destruction Form_V1 on project completion. 

This form asks Lead End Users to confirm that the Dataset and all copies (including any hard or soft copies) have been deleted or destroyed upon project completion. ISSDA kindly ask that this form is completed and returned to ISSDA no longer than 7 days after the Project Expiry Date.

ISSDA will send a copy of the form out to Lead End Users 30 days prior to project completion.
