Elective Medical forms
Step 1 :Read form to check what information is required from you, especially if you need vaccination records.
Records of vaccinations are given by either your Irish GP or the HSE and should be available on (opens in a new window)https://www.hse.ie/eng/health/immunisation/whoweare/requestrecord.html
Step 2 : Once you have all necessary information together submit your form only (opens in a new window)student.health@ucd.ie subject box “Elective Medical Application“
Step 3 : Your form will be reviewed by a doctor and if suitable, we will contact you to offer an appointment. To confirm the appointment you will need to pay a deposit of €20 (non refundable).
Some forms cannot be completed by UCD student health service doctors and may need an occupational doctor assessment. Reception will let you know if this is the case, see Occupational Doctor information below.
Step 4 You must let reception know how many separate applications need completion when booking your appointment ( Max 4). There is an additional charge for each application, which needs to be booked in advance. Additional applications cannot be processed on the day without advance notice.
Step 5 On the day of your appointment, please bring all relevant vaccine records and completed documents. If you forget, we will need to make an additional appointment and extra charges will apply
Step 6 Some elective applications will require extra vaccinations or blood tests. The doctor will arrange for you to have these. The additional costs are listed below

Remember to organise your application in good time, as there may be unavoidable delay getting records or awaiting blood test results before the form can be fully signed off
